The Oconomowoc Area School District requires all athletes to have on file, a current WIAA physical.

To participate in any athletic activity, including tryouts, camps, and practices, students must have provided Silver Lake with a physician and parent signed WIAA physical form.

Your child must have a physical dated AFTER April 1, 2023, to participate in athletics during the 2024-25 school year.

To view if your child has a current athletic physical on file, log onto your Family Access, select your student, select the Health Info tab (left side of screen), select Display Option, select Show Physical Exams.  All athletic physical will be listed, if any.
