The Oconomowoc Area School District requires all athletes to have a current Physical Clearance Form on file. The Physical Clearance Form must be dated after April 1, 2024, to participate in athletics during the 2025-26 school year.
Follow These Steps to Confirm If Your Child Has a Current Physical Clearance Form on File:
- Log in to your Skyward Family Access account
- Select your child's account
- Select the Health Info tab on the left
- Click the Display Options link, then select the Show Physical Exams link
- If your child has a physical exam on file, the information will be listed
